not sure about scary... but when it comes to surprise/shock it definitely goes to the russian movie "the green elephant", in russian "Зелёный Слоник"

Blair Witch Project - But only because I was then picking somebody up about 10 miles into a very foggy marsh area and it was filled with feral dogs who kept trying to get at me.

Hard to say what “scary” really means, but Hereditary is easily my #1. No jump scares, but the tension is brutal.

If we’re talking disturbing — A Serbian Film still wins. Nothing’s topped that yet.
